All children must be between the ages of 4-10 years and a resident of either
Broken Arrow or Coweta.
All necessary documents must be in place before sign up is confirmed.

Filling out this form is NOT A CONFIRMATION of participating in the holiday Shop-With gift program.
Once your documents are reviewed by staff you will get an email with a confirmation number and instructions.
If there are documents missing or incorrect you will have to submit those before we can review your file.
We will review and sign up clients in the order that they were received and all documents are in place.
